2. Description of object:
   Cite number of objects, size, shape, color, brightness, sound, smell, etc.








3. Position of observer, inside, outside, still, moving, graphical location.



4. Type of observation, naked eye, binoculars, still or movie camera, other optical device.



5. Direction object traveling in when first observed. Any landmarks.



6. Angular Elevation of object, if too difficult then estimated altitude. Also observers distance to object.



7. Changes in movement, speed, course.



8. Weather conditions, clear, moving clouds, hazy, foggy, mist, etc.



9. Nearby objects such as high voltage lines, telephone poles, buildings, marsh, river, swamp, reservoir, buildings,
   chimneys, steeples, spires or masts, airfields, power plants, factories, or lighted areas, etc.






10. Was this reported to anyone or any agency, press, police etc. and to whom and date?



11. Other witness and names if known.











12. Any other comments that would be helpful.
